Additional NHS Blood Testing – Patient Opt-Out Form

As part of NHS service development, quality improvement, service evaluation, or approved research programmes, additional laboratory testing will be performed on blood samples already taken for your routine care.

These additional tests:

• do not require an extra blood sample;
• do not affect your routine clinical care;
• are carried out in accordance with NHS policies and applicable legal and ethical requirements.

If you do not wish your blood sample to be used for any additional NHS-approved testing beyond the tests requested for your clinical care, you may opt out by completing this form.

I understand that:

• my routine blood tests requested by my clinician will still be carried out;
• opting out will not affect my medical care or treatment;
• I can change my decision at any time by informing the practice.
